Logging user actions in database

We're improving our database to record which user created Steps, Sections, Roles, and Teams. This improvement will prepare our system to display activity logs across the platform in the future to help answer the following questions:

1. Who added this step?

2. How many steps has each team member added?

3. Who changed this step last? What changes did they make?
